The Care Coordination team at PVMC is responsible for the coordination of patient care. We provide the following services to our hospitalized patients:

* Patient Assessment
* Discharge Planning Assistance
* Advance Directive Education
* Utilization Review to ensure all hospitalization criteria is met

When needed, we also do mental health assessments. We have knowledge of community resources and can refer you to many community agencies and services. Our Care Coordinators are also available to the community to answer any questions and provide resource phone numbers. The Care Coordination phone number is 303-498-2170, which is checked frequently during working hours.

The Care Coordination team works with an interdisciplinary team, consisting of Nursing, Pharmacy, Physical Therapy, Dietary, and the Chaplain. The team discusses high risk patients daily, their individual plan of care during their hospitalization and their potential discharge needs. This coordinated effort is most important in continuity of care and in understanding our patients and their needs.

The Coordinators assist patients and their family in facilitating plans if they need continuing care in long term hospitalization, rehabilitation, nursing home placement, assisted living, home care or hospice.

Our department consists of Master Level Licensed Social Workers and Registered Nurses, some of which have their Case Management Certifications (CCM). We are available seven days a week to assist with patient coordination.
